Job Description
Required Education:
• High School Graduate
Required Qualifications/Skills/Experience:
• Experience working in a healthcare, case management, or patient coordination environment is preferred
• Strong written and verbal communication skills
• Strong analytical and problem-solving skills
• Ability to build effective working relationships with patients, case managers, and facilities
• Proficient in Microsoft Suite of Products including Word, Excel, PowerPoint, and Teams
• Ability to handle multiple tasks and work in a fast-paced environment
Overview:
• This is a 100 percent onsite role based in Atlanta, Georgia
• Candidate must be within daily commuting distance from the facility
• Free parking is available at the location
• Dress code is Business Casual
• Contract duration is 3 months
• Two positions are open
Job Summary:
• The Outside Referral Coordinator will support case managers and members in skilled facilities
• This role focuses on coordinating appointments, transportation, and handling day-to-day non-clinical tasks
• The worker will act as a support bridge between members, facilities, and care teams
Job Responsibilities:
• Assist members in skilled facilities with arranging transportation to and from appointments
• Schedule and coordinate medical appointments for members
• Assist Case Managers with day-to-day non-clinical administrative tasks
• Communicate effectively with members, providers, and internal teams
• Track and follow up on appointments and related coordination activities
• Maintain accurate documentation using Microsoft Office tools
