Job Description
Required Education:

• Valid Pharmacist License in Florida - please screenshot and put at the top of resume



Required Skills/Experience:

• At least 4 years of experience as clinical MTM pharmacist. (Management experience preferred)

• Familiar with driving Part D Star measures. Specifically, SUPD, SPC, and CMR.

• Understanding of pharmaceutical and medical vocabulary

• Clinical experience in medication review.

• Excellent verbal and written communication skills

• Must be tech-savvy



Preferred Skills/Experience:

• Management experience preferred

• Experience using Salesforce preferred

Licensed Clinical Pharmacist Overview:

• The pharmacist will be responsible for medication therapy management (MTM) services, including outbound calls for comprehensive medication reviews (CMR) and delivering in-depth consultations on statin medication use per AHA guidelines.

• The pharmacist will transcribe patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application.

• The data to be transcribed will be either hard-copy or available from a Client application.

• The data entered by the pharmacist will be used to create a standardized document that will be mailed to individual patients.

• The pharmacist is a member of the MTM product team.



Job Duties:

• Conducting outbound phone calls. (100 calls per day)

• Completing comprehensive medication reviews.

• Conducting medication therapy management services.

• Accuracy and speed in entry of patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application

• Attend training sessions to acquire/enhance skills related to Essential Job Responsibility
