Center Nurse Overview
• The Center Nurse is a clinical care team member operating within a Value-Based Care model that prioritizes patient health outcomes and long-term wellness
• Supports patient health through clinical assessments, chronic condition management, evidence-based standards, guidelines, and care pathways designed to prevent complications
• Drives quality care and collaborates with the care team to reduce avoidable hospitalizations and improve patient outcomes
• Fosters a welcoming, engaging environment through effective communication and delegation while empowering the care team to deliver high-quality patient care
• Supports walk-in and telephonic triage, skilled nursing procedures, medication management, preventative care, patient education, and onsite testing
• Serves as a clinical resource and voice within daily care team meetings while supporting nursing staff, clinical team members, and providers
• Reports to the Director of Clinical Support

Job Duties
• Support patient assessment and prioritization for walk-in and telephonic triage to ensure patients receive the appropriate level of care and avoid unnecessary hospital visits
• Execute protocol-driven chronic condition management
• Perform skilled nursing procedures, including wound care and medication administration, including vaccines
• Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care in person, over the phone, or in group settings
• Conduct medication reviews during nurse-led visits to ensure patient safety and adherence
• Conduct onsite testing, including point-of-care labs, EKGs, and specimen collection
• Use patient data to identify and close gaps in preventative screenings while ensuring compliance with state scope of practice
• Utilize standardized protocols for medication management, prescription renewals, and prior authorizations
• Ensure clinical communications are managed, tasks are completed, and patient visit encounters are closed in a timely manner
• Delegate to medical assistants and phlebotomists as needed to support patient flow and efficiency
• Support the coordination, integration, communication, and implementation of nursing practice and standard operating procedures
• Identify barriers to optimal care, including physical, cognitive, language, literacy, socioeconomic, cultural, and behavioral barriers
• Create a welcoming and engaging environment that meets the needs of patients, communities, families, and teams
• Deliver an exceptional patient experience through service, responsiveness, and respectful care
• Serve as a key clinical voice in daily care team meetings to align on patient priorities and clinic operations
• Serve as a support and resource for nursing staff, other clinical staff, and providers
• Ensure clinical compliance with Patient Safety initiatives and reporting
• Provide emergency care in accordance with organizational procedures
• Engage in self-evaluation and participate in activities that promote professional growth and development
• Seek, acquire, and maintain current nursing knowledge and competence to fulfill responsibilities as a Nurse
• Maintain awareness of current patient-centered, evidence-based practice and consider applicable research data in patient care delivery
• Fulfill the obligation to identify and appropriately report professional misconduct
