Job Description
Education Required:

• Bachelor’s degree in public administration, public health, or a closely related field



Skills Required:

• Bi-lingual in English and Spanish highly preferred

• A working knowledge of the principles, practices, and techniques of public health

• A working knowledge in the development and implementation of public health promotion activities

• A familiarity with chronic disease including diabetes, heart disease, Alzheimer’s disease, and dementia

• Ability to make written and oral presentations

• Ability to establish and maintain effective working relationships with superiors, associates, and public groups



Job Responsibilities:

• Outreach to the Health Equity Zones (HEZ), with a focus on Providence, Central Falls, and Pawtucket, to invite them to engage in efforts to organize and schedule a community education session in each of the communities.

• Offer a minimum of 15 Evidence-Based/ Disease Awareness presentations in both English and Spanish within a public housing authority, community-based organization, or faith-based community, preferably within a Health Equity Zone. Relevant educational opportunities will be posted to the Community Health Network (CHN) website.

• Implement evaluation tool for community education programs.

• Complete the Community Engagement Report identifying number of individuals reached through community education programs.

• Participate in outreach and action planning to support municipalities that are currently convening stakeholder groups in the development of Age- and Dementia-Friendly Community Action Plans that address social determinants of health for aging populations across Rhode Island.

• Attend community events to raise community awareness and provide education to community members.

• Conduct a caregiver support group for Spanish-speaking caregiver with community health center partners.

• Build partnerships with Health Equity Zone leaders and backbone agencies to coordinate one community event focused on increasing awareness of ADRD and supporting the Hispanic community living in Rhode Island.

• Promote information about ADRD topics to a broad audience using multi-media strategies including social media, podcasts, radio interviews, etc.

• Consistent and ongoing community outreach to key partners to identify opportunities for collaboration and strengthening existing partnerships.

• Participate in components of evaluation of the ADRD Program.

• Attend professional workshops, seminars, trainings, etc.

• Participate in ongoing integration efforts of the Division of Community Health and Equity and the Center for Chronic Care and Disease Management.

• Assist the Program Manager with reporting, success stories, and grant writing.

• Assist the Program Manager with the coordination and implementation of the work plan, including tracking program activities, evaluation, goals, and objectives, to ensure performance measures are being achieved.

• Perform other duties as assigned by supervisor.
