Job Description
Required Education:
• High School Diploma or equivalent
Preferred Education:
• Bachelor’s Degree preferred or a minimum of 3 years of experience in Case Management or a directly related field
Required Qualifications/Skills/Experience:
• Minimum 3 years of experience in Case Management or a directly related field
• Experience with chronic disease management programs
• Experience working with patients managing conditions such as diabetes, renal disease, respiratory conditions, or similar chronic illnesses
• Proficiency with Epic EMR system
• Excellent communication and interpersonal skills
• Strong collaboration and teamwork abilities
• Ability to multitask and manage multiple patient cases simultaneously
• Strong organizational and follow-up skills

Job Summary:
• The Case Manager will support patients with chronic health conditions through proactive outreach, care coordination, and disease management activities. This role focuses on improving patient outcomes by connecting members with appropriate healthcare resources, scheduling appointments, and supporting ongoing care plans.
Job Responsibilities:
• Conduct outreach to patients with chronic health conditions
• Coordinate and schedule appointments with physicians, registered nurses, and PharmD staff
• Provide educational materials and community resources to patients
• Support disease management programs focused on chronic conditions such as diabetes, renal disease, and respiratory disorders
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care
• Document patient interactions and care activities within Epic
• Monitor patient progress and follow up as needed
• Maintain accurate and timely records in accordance with organizational guidelines
• Assist patients in navigating healthcare services and available resources
