Job Description
• Required Level of Education: Bachelors degree



• Top Three things Worker will be doing: Managing community hub resource desk vendor, liaising with additional community-based organizations providing on-site service, improving workflow processes in collaboration with local and vendor staff



• Top Three Skillsets needed: Program coordination, process improvement, grant management



• Remote Options (Fully Remote, Partially Remote, Client Onsite): Partially Remote

• Will the worker need to be Remote local to the office or Remote anywhere in the US: Remote local in Colorado

• Percentage of Travel? Limited travel outside of Colorado

• Specific Details around travel including locations, frequency, different locations throughout the same day, etc. Aurora Centerpoint, Lakewood, and Westminster clinics (in addition to remote and community-based work).

• Expected Shift: M-F 8-5pm

• Interview Process: Phone and/or email screening, panel interview, final interview





Position details:



• Will provide dedicated operational management and oversight of community based resources at select Client clinics by supporting the integration of community and social health resources alongside operation, including 3 new Medicaid hub sites.

• Program management of community resource desks at select clinics, including onsite CBO partners such as Benefits in Action and other contracted CBOs

• Supervision of daily workflows and ongoing process improvement to support consistent execution, including facility and equipment access, recommending staffing levels, data tracking, escalation pathways, etc.

• Management of community benefit–funded resources, including emergency food bags and other on-site supports and resources.

• Stakeholder management of CBO’s, including related philanthropy, and collaboration between CBO’s, Community and Social Health, and market level clinic operations teams.

• Support for member focused initiatives and HR1 community engagement activities, particularly as they are implemented on-site in Client clinics.
